IMPORTANT INFORMATION ABOUT COVID-19 REQUIREMENTS:
We are looking forward to hosting the 28th Annual Paul J. Hartman SALT Forum on October 27-29, 2021. As mentioned previously, we are holding a robust in-person program here in Nashville and will also provide it virtually for those unable to travel.
If you plan to attend in-person, the requirements are as follows:
1.You must provide proof of full vaccination or proof of a negative COVID-19 PCR test result taken within 72 hours of the first day of the Forum. (Digital or printed proof of the test results must be dated and time-stamped.)
2.Masks are required unless you are seated and socially distanced at least 6 feet apart.
3.You will be asked to answer the following questions, and if any answers are “yes” you will not be able to attend.
*Have you been in direct contact with anyone with COVID-19 in the last 14 days?
*Have you had any COVID-19 symptoms in the last 14 days?
*Are you currently awaiting the results of a COVID-19 test?
*Have you tested positive for COVID-19 in the last 14 days?
4.All attendees must sign a waiver. The language will likely read:
The novel coronavirus, COVID-19, is extremely contagious and is believed to spread mainly from person-to-person contact. The Paul J. Hartman SALT Forum has put in place preventative measures to reduce the risk of spread of COVID-19. You acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you may be exposed to or infected by COVID-19 by attending this event. You accept sole responsibility for, and hereby release any claims against Paul J. Hartman SALT Forum arising out of, any injury or illness or any other loss, claim, liability, or expense of any kind that you incur as a result of attendance at this event. You also agree to abide by other more specific precautions such as the use of hand sanitizer, face masks, temperature checks and distancing, whether required by government, specified by venue, or recommended by Paul J. Hartman SALT Forum.
Registrants who are unable to attend the Forum will receive a full refund less a $50 cancellation fee if the Forum is notified via THIS FORM by October 20, 2021. Any cancellation requests received after that date will not be approved, unless proof of Covid-19 infection being a factor is provided.
